In general, O'Reilly's is a superb establishment with excellent British-Isles food and an impressive selection of fine beers. The atmosphere, complete with a stunning mural of Irish writers, is first rate. This place is just one step away from being one of the premier drinking spots in San Francisco (if it isn't already_ - but that step involves more consistency with service.
I visited O'Reilly's three times while in San Francisco with my wife. I quickly discovered that one needs to be near the bar to get served in a timely manner. Make the mistake of sitting outside or in a corner and you will not get the time of day. During the first visit, on a Sunday evening, it took me a full thirty minutes to receive my first pint. Once I was finally acknowledged, I enjoyed wonderfully fresh beer (Anchor and Bear Republic) and an excellent lamb shank meal. Wednesday evening we sat at the bar, and then the service was fast and friendly, and I enjoyed the best fish and chips I had in years. On Friday we sat in the corner by the Irish Writers mural and once again getting served was like pulling teeth. In addition, our bill was messed up and we received a check before I was even done drinking. That was also the one time the food fell short - stick with the Irish fare. The calamari was undercooked and limp, and the "sauce" that came with it tasted like an evil mixture of ketchup and blood. Thank goodness the beer was as great as ever and managed to wash it down without incident.
Overall, I would recommend O'Reilly's strongly for its great beer, Irish menu and atmosphere. The servers are all friendly folk but need to work on their peripheral vision.

Wonderful place to hang out & have a beer. Homey atmosphere - if you can get to the bar! ;-) Our 1st day in SF we went here, no room at the bar, & it took our server a little while to get our beers and food order. We hung around anyway. Went back 2 days later, managed to get seats at the bar, got blitzed (in a good way)!
Our last full day in SF was dinner here. This time we sat "next to" the poets & playwrights painted on the wall (I sat "next to" Oscar Wilde). Murals seem to be a SF hallmark, and this place was no exception. Hubby stuck with Anchor while I stuck with Bear Republic's Racer 5 - both AWESOME! I felt as if I were in a real Irish pub - everyone was friendly plus it felt cozy and homey.
